Do you have a good enquiry form?

I've used a few from various sites but I haven't really found one I like, the're all really messy. Does anyone have a link to a nicely laid out enquiry form which can be easily adapted?

What all did you need on it? I just started a contact form without using tables actually.

I have two that I have used, and are very cool.

http://www.dustindiaz.com/ajax-contact-form/ (a very sweet ajax contact form, also CSS & tableless)

I hope you like them, as they are both very cool forms.
