code for accumulating credible data

A simple question that could help a whole lot
Good day to all you techies out there.
I am working on a web site project and need a tip on one simple thing...
The idea is to accumulate credible data. I've found scripting for just simple polls, but they are no better then a joke.
What I was looking for, and I'm guessing it would be a common & simple fix for anyone who knew how, would be to make a poll, with additional input sections for name, email, and comments, still with a way to quantify all the yes or no's into one figure after all i|s said and done.
To accumulate and document legitimate, useable data. What would be the best way?
Any help would be greatly appreciated, thanks.

Do you know any PHP? You will need to know a dynamic language for this and know how to store information in a database.
For this you can use dynamic languages such ASP and PHP.
I will move this to the PHP section as you will most likely use it.

Alright.... so honestly, how long do you take it would think to learn this? I'm sure i've seen functions exact to this performed all over the web before..
Or at the very least, could you hazard a guess at how much it would take to get someone to do it for me?

May take a month to understand JavaScript and PHP basics (Longer if you don't even know HTML), then a few weeks after that to code something like this.
It would be probably be best to pay someone for this, personally if I were to something like this I'd want around £200 (~$400).
If you want someone to do it for you post in the job opps section.

spread the word about that one in the name of free knowledge/not charging 2-400$ for what can be bought for around 30.
