Can this approach work for my form or any other suggestions?

Hi. This is what I want to do:

Basically, I would like to have a form on my site that, once completed by a subscriber, will trigger another form (long survey) to be downloaded automatically from an autoresponder, for the subscriber to fill (in and send back results to site).

Now, the first part (of setting the first form that will trigger the responder message) is the easy part.

The difficult part is to trigger the second form so that it could be downloaded from the responder. Will it serve my objective if I prepare a standard form with dreamweaver (say in the form of an HTML web page) and then add it as a message on the responder? Please note that my responder does allow HTML messages.

The reason I want to do it this way (as opposed to posting my survey form on my site), is because I want to collect the details of those persons completing the form to subesequently contact them. I don't want to make this survey form open to all.

Also, is there another way of achieving this (for example without using autoresponders)?
Please note that I am not an IT professional person and I thereofre would be grateful for plain English.

I personally don't know of a way to do that in plain HTML. If I wanted to do it, I might try Javascript or DOM, but you might need to do it server-side anyway for people who have scripts disenabled.
