Online pdf Forms via SSL

Firstly I apologise is this is in the wrong Forum but when you read my question you'll probably appreciate my dilemma.

I want to create a pdf Order Form that users can either download and complete offline and then post/fax or alternatively complete online and email to us securely or send the information to us in a secure manner. Our web host provides a secure doc or cgi server facility.

im not sure you can "create" a pdf dynamically online....... (i could be wrong) the way id do it is to create a user system where members can fill in an online php form and order via email that way... concerning the pdf file... if you really need to protect it you could look into .htaccess & .htpasswd files to protect the folder containing said pdf.

I do apologise, the wording of my question left a lot to be desired.

I want to place a pdf from online and make it available so that customers can (a) complete it online and then print it out (b) print it out and complete it manually or (c) complete it and send the information to me securely.

I can make the form I just don't understand how to apply the technology that will keep the information secure and then give me access to it
