help with asp contact form?

Hi there, does anyone have a simple asp contact form that I can manipulate for my own? I have a contact form for my website that I cut and pasted, and tried to tweak, but I just don't know enough about it to do what I need to do... Here is the link if you'd like to look at it, it would be a great help. The face of it has the questions that I need to ask, but the script is completely messed up.

This require a basic knowledge in ASP and MySQL or MSSQL!

Upon submitting the form, where do the information goes? Database? Auto-Generated Email to customer??
If it goes to the database for storage, you will need to learn some ASP tutorial here:

The html needed to be changed for the multiple radio buttons, but does it also need to be changed on the .asp? Where? It's still not going to my email...
