I need PHP Contact & Signup forms?

i dont hve database*. so i want through my mail with viewer IP details also.

plz tell me how to use tht. im using dreamweaver 8.


When viewer submit forms i want 2 receive through mail.

anybody try 2 help me. its urgent.
