1. A good title tag will describe the main copy of the page that it is heading.
2. Use google webmaster tools - this will warn you of title tags that are too long or repeated in your site.
3/4. Don't repeat keywords, you will not be in google's good books for this and hinder your chances of getting up the ranks.
