I am having trouble with a form I am working on.

It will only post the first field on the submit button and I am not sure why.Can anyone take a look and let me know what I am doing wrong?

It is supposed to send all information to my email. It only sends the first name info. I have gotten forms to work correctly like this before, and I can't see a difference in the code. If anyone knows what I am doing wrong, please let me know.
