OK - a bit of the info above is incorrect...

I went back to the page with the form and started over (as opposed to using F5 or Ctrl+F5 to test changes) and reposted the form and it stops giving the error if I comment out both setcookie lines.

So what the heck is wrong with my setcookie lines?
